New APC Final Assessment Templates - Simpler, faster, more straightforward
From 1 December 2008 new templates will be available for all UK APC candidates. This means submitting final assessment submissions will be much simpler, faster and more straightforward.
What's New?
The templates for APC final assessment submissions are now available in one user-friendly Excel workbook. There is just one document to capture all experience from enrolment to final assessment with added functionality to reduce repetition. Information entered is copied across the workbook to save time and a help function provides useful tips to assist candidates with their submission.

All candidates completing submissions for final assessment from Session 1 2009 onwards need to use the new templates. All assessments will be under 2006 APC guides from Session 1 2009.
You can now download the new templates.
Candidates reapplying for final assessment need to re-submit their existing submission (as per your referral report) and complete the following new templates:
- Candidate declaration
- Candidate checklist
- Referral deficiency report
- Referral PD deficiency report
